Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and vital signs monitoring

• Develop and implement individualized care plans, working closely with the healthcare team to ensure comprehensive care

• Conduct assessments, document patient progress, and communicate effectively with colleagues and family members

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

• Collaborate with other healthcare professionals to ensure efficient and effective care delivery

Requirements:

• Current licensure as a Licensed Practical Nurse (LPN) in the state of Illinois

• High school diploma or equivalent required; associate's degree in nursing or related field preferred

• Ability to lift, transfer, and reposition patients; must be physically able to perform duties without assistance

Compensation:

$$33 - $38 HOURLY
