We are currently seeking an Home-based Nurse (LPN/RN) in Anchorage, Alaska.

Hope Community Resources, Inc. is a non-profit disabilities service provider based in Anchorage, Alaska. Founded in 1968, our company provides services and supports to people of all ages and abilities who experience an intellectual, developmental, or other disability, traumatic brain injury, or mental health challenge.

This is a 40 hour Shift: Monday-Wednesday, and Friday- Saturday.
Responsibilities include:

• Ensures proper and consistent medical care for support recipients in accordance with physician orders, established care plans, and applicable regulations.
• Manages medication provision, administration oversight, documentation, and related compliance requirements.
• Monitors support recipients' health status and psychosocial needs on a regular basis to identify changes in condition..
• Coordinates and collaborates with team members to promote optimum health, wellness, and continuity of care.
• Functions independently within the scope of licensure, utilizing clinical judgment to address health concerns and provide timely guidance

Requirements

Education and Experience

• Associate degree in Nursing required, or equivalent nursing program.
• One year of nursing experience preferred. Experience supporting individuals who experience developmental disabilities preferred.
• Current State of Alaska Licensed Practical Nurse or Registered Nurse license required and must be maintained throughout employment.

Salary : $30 - $43
