What are the responsibilities and job description for the Social Worker position at Hood Memorial Hospital?
JOB SUMMARY:
The Social Worker works in collaboration with the patient and clinical staff to provide an efficient and compliant service following patients before and after discharge. Demonstrates commitment to the philosophy and objectives of discharge planning, community - and population health management, palliative care, and readmission reduction efforts. The Social Worker provides appropriate healthcare services, resource identification, and follow up to patients during admission and post-discharge as indicated by the patient, interdisciplinary staff, social work assessment, and/or facility policy.
EXPERIENCE, KNOWLEDGE AND SKILL:
1. Previous Experience Preferred
a. Two (2) years of Social Work experience is preferred.
b. Previous hospital Social Work experience with an emphasis on discharge planning experience preferred.
c. Knowledge of hospital utilization review processes.
2. Specialized or Technical Education
a. Master’s degree in social work required from a program accredited by the Council on Social Work Education (CSWE).
b. Credentialed as Certified Social Worker (CSW), Licensed Master Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW) by the Louisiana State Board of Social Work Examiners (LABSWE).
c. Knowledge of Prospective Payment System, Peer Review Organizations, Medicare, Medicaid, third-party payors, community resources.
d. Knowledge of quality improvement methodologies preferred.
