Responsibilities:
- Provide direct patient care, including assessments, treatments, and medications as prescribed by physicians
- Develop and implement individualized care plans, taking into account patient preferences, needs, and goals
- Collaborate with interdisciplinary teams to ensure comprehensive care, including attending care conferences and contributing to case management
- Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care transitions
- Perform administrative tasks, including documenting patient information, charting, and reporting
- Participate in quality improvement initiatives and performance improvement activities to maintain high standards of patient care
Requirements:
- Current licensure as a Licensed Vocational Nurse or Registered Nurse in the state of Texas
