Job Summary

Duties
- Assist residents with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
- Provide medication administration and monitor vital signs in accordance with care plans
- Support patients with memory care needs, including Alzheimer's and dementia care
- Prepare nutritious meals tailored to individual dietary requirements and assist with feeding as needed
- Observe and document patient conditions, behaviors, and progress through accurate record-keeping
- Utilize assistive devices such as Hoyer lifts and perform heavy lifting when necessary to ensure safety during transfers
- Maintain a clean and safe environment by performing light housekeeping tasks including cleaning and sanitation
- Follow HIPAA guidelines to ensure patient confidentiality and privacy at all times
- Collaborate with healthcare professionals to implement personalized care plans and support overall patient well-being
- Respond promptly to emergencies or changes in patient condition by following established protocols such as CPR or first aid procedures
Skills
- Proven experience in home & community care, assisted living, nursing homes, or hospice settings preferred
- Knowledge of patient monitoring techniques and vital signs measurement
- Familiarity with Alzheimer's, dementia, autism spectrum disorders, and developmental disabilities care strategies
- Ability to assist with ADLs effectively while maintaining dignity and respect for residents
- Strong communication skills for working with patients, families, and healthcare teams
- CPR/First Aid certification required; additional certifications such as HIPAA compliance are advantageous
- Physical ability to perform heavy lifting and assist with transfers using equipment like Hoyer lifts
- Compassionate attitude with patience for working with vulnerable populations across diverse age groups
- Ability to follow detailed care plans accurately while adapting to individual needs
