Position Summary

Provides activities of daily living (ADL) care and support for residents. Assists residents in meeting their physical, personal, and daily needs by performing a range of tasks in accordance with the residents’ care plan and applicable regulations.


Key Responsibilities

  • Provides direct care and support to residents, including but not limited to:
  • Personal care assistance: bathing, dressing, grooming, hair care, nail care, and oral hygiene
  • Mobility assistance: ambulation, wheelchair transport, and safe transfers to and from chairs, beds, and vehicles
  • Meal assistance and preparation as assigned
  • Resident transportation, including transport to medical appointments and, where applicable, community errands such as grocery or personal shopping
  • Observes and documents resident information accurately and completely; reports changes in resident condition or needs to the supervising nurse or appropriate supervisor in a timely manner.
  • Assists with departmental administrative duties as directed, including filing, charting, and documentation.
  • Performs additional duties as assigned by supervisor or department leadership.


Qualifications

  • Certified Nursing Assistant (CNA) in good standing with the applicable state registry.
  • Genuine interest in working with older adults; demonstrates compassion, patience, and empathy in interactions with residents and families.
  • Ability to assess resident needs and respond proactively and appropriately.
  • Strong communication and organizational skills; ability to communicate clearly with residents, families, and coworkers and manage sequential tasks effectively.
  • Ability to work independently and exercise sound judgment without direct on-site supervision.
  • Valid driver's license; must meet Homewood Living Ministries' approved driver requirements to transport residents.


Physical Requirements

Work requires frequent standing, walking, bending, and physical activity throughout the shift. Must be able to lift, assist, and/or support residents during transfers and mobility tasks in accordance with established safety protocols. This may include lifting up to 30 pounds and participating in team lifts or use of mechanical lifting equipment. Requires the ability to respond quickly in emergency situations and provide physical support to residents as needed.
