HomeWell Care Services is a growing home care agency serving individuals and families across Duval, Nassau, and Clay Counties. We are looking for a driven, detail-oriented Junior Care Manager to join our team and help us deliver seamless, high-quality care to the clients who depend on us.
What You Will Do
∙ Coordinate directly with caregivers and client families to ensure continuity and a consistent standard of care across all active cases
∙ Manage caregiver scheduling, including filling open shifts personally in the field when coverage cannot otherwise be secured
∙ Monitor client care plans and document changes in condition, flagging clinical concerns to the supervising RN and Agency Directors
∙ Serve as a primary point of contact for clients and authorized representatives regarding day-to-day care coordination
∙ Maintain accurate and timely records in WellSky, ensuring compliance with payer and regulatory documentation standards
∙ Support intake, onboarding, and ongoing case management across all active service lines
∙ Assist with caregiver compliance tracking including scheduling, documentation deadlines, and training requirements
∙ Collaborate with the Agency Directors on operational priorities, escalations, and quality improvement initiatives
What We Are Looking For
∙ Prior experience as a home health or home care care manager is required
∙ Active HHA or CNA certification with genuine comfort performing hands-on caregiver duties when needed
∙ Demonstrated experience with WellSky scheduling and EVV platform
∙ Proficiency in Microsoft 365 including Word, Outlook, and OneDrive
∙ High emotional intelligence, genuine compassion, and strong interpersonal skills are essential. Our clients and their families are navigating some of the most vulnerable seasons of their lives and this role requires someone who meets them with patience, warmth, and professionalism every time
∙ Strong written and verbal communication skills with a professional, family-facing presence
∙ Highly organized with the ability to manage multiple active cases and shifting priorities
∙ Reliable transportation and valid Florida driver’s license
∙ Ability to pass a Level 2 background screening
Preferred Qualifications
∙ Experience working with veteran clients or within the VA Community Care Network
∙ Familiarity with Florida Medicaid waiver programs and documentation requirements
∙ Bilingual candidates are encouraged to apply
