Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor patient health by checking vital signs and observing for any changes in condition
- Administer medications accurately following care plans and HIPAA regulations to ensure patient safety
- Prepare nutritious meals tailored to dietary restrictions and preferences while maintaining cleanliness in the kitchen
- Support clients with mobility using equipment such as Hoyer lifts or other assistive devices when necessary
- Maintain detailed documentation of care provided, patient observations, and any incidents or concerns
- Perform light housekeeping tasks including cleaning, laundry, and organizing to promote a safe and comfortable environment
- Collaborate with healthcare professionals to implement personalized care plans for clients with conditions such as dementia, Alzheimer's, autism, or developmental disabilities
Skills
- Proven experience in home health, assisted living, nursing homes, or group home settings
- Knowledge of patient monitoring techniques and vital signs assessment
- Strong understanding of HIPAA compliance and confidentiality standards
- Ability to perform CPR and First Aid confidently in emergency situations
- Experience working with individuals with disabilities or special needs such as autism or dementia care
- Skilled in medication administration and managing care plans effectively
- Physical ability for heavy lifting and assisting with mobility aids like Hoyer lifts or resident transfers
- Compassionate communication skills to foster trust and rapport with clients and their families
- Familiarity with social work principles related to long-term care and community resources
