Overview:
Seeking a dedicated RN Case Manager to coordinate and deliver in-home nursing care. This role involves managing a caseload of approximately 45 clients, performing clinical assessments, care planning, and acting as a liaison among care teams. Includes field nursing tasks such as wound care, glucose checks, and medication setup.
Minimum Requirements:
Valid and unrestricted Nurse (RN) license in the State of MinnesotaBLS certification & CPR (required)
Oasis experience not mandatory but preferred
Strong Clinical skills and the ability to conduct comprehensive assessments
Minimum one year of nursing experience (home health or hospice preferred)
Exceptional communication and interpersonal abilities
Key Responsibilities:
Perform comprehensive assessments and maintain individualized care plansCoordinate with providers and supervise unlicensed staff
Complete reauthorizations and maintain accurate documentation
Provide health education and support to clients and families
Deliver hands-on nursing care (vitals, MARS/TARS, discharge planning, etc.)
