What You’ll Do
Relationship Building
- Develop and sustain strong partnerships with hospitals, rehabilitation centers, senior living communities, clinics, and community groups.
- Serve as the primary external ambassador for Homewatch CareGivers of Northwest Indiana.
- Cultivate deep trust with referral partners through consistent presence, professionalism, and follow‑through.
Field Outreach
- Conduct daily, high-impact outreach to Skilled Nursing Facilities, Assisted/Independent Living communities, hospitals, physicians’ office, rehab centers, etc.
- Maintain a predictable field cadence to strengthen partner relationships and uncover new referral opportunities.
- Monitor referral quality and collaborate with Operations to support lead-to-client conversion and client onboarding.
Referral Management
- Manage, nurture, and grow referral partner relationships through consistent communication and exceptional service.
- Track referral activity, identify trends, and provide timely follow‑up to all partners.
- Partner closely with Operations to ensure smooth onboarding and minimize any adverse client experiences.
- Troubleshoot potential barriers early to protect the client journey and enhance satisfaction.
Reporting & Intelligence gathering
- Maintain accurate documentation of all outreach activity within the CRM.
- Provide summaries detailing field visits, opportunities, feedback, and market intelligence.
- Share insights on local competitor activity, facility updates, and emerging community needs.
Marketing Collaboration
- Coordinate with marketing to supply photos, stories, and timely field updates for campaigns and social content.
- Offer field‑level insights that support messaging, event planning, and strategic marketing initiatives.
- Represent the organization at community events, senior fairs, chamber gatherings, and networking functions.
What You Bring
- 2–4 years of experience in healthcare outreach, community liaison work, referral management, business development, or similar high‑touch relationship roles.
- Strong communication skills, with the ability to influence and build rapport across diverse healthcare settings.
- Comfort and enthusiasm for a field-based schedule involving regular travel across Northwest Indiana; willingness to commute to community locations 4 days a week.
- Experience partnering with internal teams to support client onboarding or care workflows.
- CRM proficiency or willingness to learn quickly.
- Valid driver’s license and reliable transportation required.
