Responsibilities
- Provide attentive supervision and patient observation to monitor health status and respond promptly to changes.
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
- Administer medications accurately following medical documentation protocols and ensure proper medication management.
- Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient care activities and medical records securely in compliance with HIPAA regulations.
- Support clients with dementia or Alzheimer’s care, memory care routines, hospice needs, and caring for individuals with developmental disabilities or disabilities.
- Operate medical equipment such as Hoyer lifts, catheters, vital signs monitors, and perform catheter care as needed.
- Prepare nutritious meals and assist with meal planning while maintaining cleanliness and hygiene standards in the home environment.
- Conduct home safety assessments, cleaning, laundry, and general household chores to promote a safe and comfortable living space.
- Collaborate with healthcare professionals regarding patient care plans and medical terminology understanding to ensure consistent quality care.
- Maintain detailed medical documentation and ensure compliance with all privacy regulations including HIPAA.
Requirements
- Proven experience in senior care, home health, assisted living, nursing homes or long-term care environments.
- Strong skills in patient observation, medication administration, vital signs monitoring, and dementia or Alzheimer’s care.
- Knowledge of medical terminology, medical records management, and familiarity with EMR/EHR systems.
- Certifications such as CPR (Cardiopulmonary Resuscitation), first aid training, or hospice care certification are highly desirable.
- Ability to assist with ADLs using tools like Hoyer lifts safely and effectively.
- Experience working with individuals with disabilities or developmental challenges is preferred.
- Excellent communication skills to work collaboratively with clients’ families and healthcare teams while maintaining confidentiality under HIPAA guidelines.
