Schedule: Tuesday–Friday, 7:45 AM–5 PM; Alternating Mondays off or 8 AM–12 PM.

Position Summary:

The Registered Nurse at Hometown Medical Group plays a key leadership role in supporting clinical operations, working alongside medical assistants to ensure high-quality patient care and clinic efficiency. This position bridges direct clinical nursing responsibilities with operational oversight, quality improvement, and coordination of complex care tasks that extend beyond routine medical assistant scope. This role is ideal for an RN who enjoys problem-solving, collaboration with providers, and contributing to system-level improvements in patient care delivery.

Key Responsibilities:

Direct Clinical Nursing Responsibilities (Primary Focus – ~70%):

• Review patient charts to identify and order preventive screenings and services that are due, including but not limited to Annual Wellness Visit (AWV) measures, health maintenance items, and age-appropriate preventive care, per clinic protocols and provider direction.
• Assist providers with in-office procedures as needed (e.g., injections, wound care, biopsies, EKGs, or other minor procedures within RN scope).
• Administer IV fluids and other IV therapies when clinically indicated and ordered by a provider.
• Administer medications, vaccines, and injections within RN scope and per standing orders.
• Perform nursing assessments, triage, and clinical follow-up as appropriate in the primary care setting.
• Provide patient education related to treatments, procedures, preventive care, and follow-up instructions.
• Serve as a clinical resource to front office staff when RN-level judgment or licensure is required (i.e. phone triage of symptom/severity, fielding complex portal messages from patients, deciding ER vs. office vs. home care, advising patients on symptom management, evaluating side effects of adverse reactions, assessing adherence issues)
• Perform nurse visits to assess wound healing progression, evaluating for infections, deciding when treatment changes are indicated.
• Assessing and documents diabetic foots exams during AWV visits.

Clinical & Care Coordination Responsibilities:

• Oversee repeat imaging requests
• Review reminders and communications from outside imaging facilities regarding repeat imaging needs (including but not limited to breast ultrasounds, mammograms, and low-dose CT scans).
• Perform chart review of prior imaging and clinical history to determine appropriate follow-up.
• Enter and route orders for repeat imaging per provider protocols and evidence-based guidelines.
• Sift through and review outside medical records, extracting relevant diagnoses, history, and testing; Document key findings and flag items requiring provider follow-up.

Prior Authorizations & Utilization Management:

• Complete peer-to-peer reviews with insurance medical directors as needed.
• Manage complex or difficult prior authorizations (PAs), including appeals.
• Assist providers in identifying appropriate step therapy options when prior authorizations are denied.

Controlled Substance Oversight:

• Assist with clinic compliance related to controlled substance monitoring.
• Oversee and document random pill counts, including coordinating patient callbacks and supervising in-office pill count procedures.
• Ensure adherence to clinic policies and regulatory requirements related to controlled substances.

Quality Improvement & Clinic Flow:

• Assist in monitoring and improving clinic flow, patient throughput, and communication between clinical staff, front-office staff, and administrative staff.
• Participate in and support quality improvement initiatives, particularly those involving patient follow-up and care coordination.
• Identify inefficiencies or gaps in workflow and provide recommendations for improvement.

Leadership Responsibilities:

• Assist with providing guidance, direction, and support for Medical Assistants.
• Assist with training, coaching, and reinforcing clinical and operational standards.
• Act as a clinical resource for Medical Assistants when questions or issues arise.

What We’re Looking For:

• Proven experience as a Registered Nurse or similar clinical role
• Strong attention to detail and a passion for patient care
• Ability to follow structured responsibilities and work collaboratively
• Ability to oversee and delegate tasks to Medical Assistants
• Active license as an RN in the state of Alabama

PTO / Holiday Pay:

80 hours PTO yearly 64 hours Holiday Pay yearly (144 hours total)
