Position Summary
The Community Relations Representative is responsible for increasing awareness of Hometown Manor Assisted Living, developing strong community and professional referral relationships, generating qualified leads, and supporting occupancy growth across Hometown Manor communities.
This position is highly relationship-driven and requires regular travel to Hometown Manor locations, hospitals, rehabilitation centers, skilled nursing facilities, physician offices, senior organizations, community events, and other referral sources throughout assigned markets.
The ideal candidate is outgoing, organized, professional, self-motivated, and passionate about helping seniors and their families find the right living environment.
Essential Duties & Responsibilities
- Develop and maintain relationships with professional referral sources, including hospitals, rehabilitation facilities, skilled nursing facilities, home health and hospice agencies, physician offices, senior centers, churches, and other community organizations.
- Conduct regular in-person visits with referral partners throughout assigned territories.
- Identify and develop new referral sources and community partnerships.
- Generate qualified leads and referrals for Hometown Manor communities.
- Represent Hometown Manor professionally at networking events, community events, health fairs, senior events, and other outreach opportunities.
- Educate referral partners and community members about Hometown Manor’s services, amenities, pricing, availability, and admission criteria.
- Work closely with community leadership to identify occupancy needs and develop targeted outreach strategies.
- Assist with tours, prospective resident follow-up, and family communication when needed.
- Maintain consistent communication with referral sources and nurture long-term professional relationships.
- Track outreach activity, referrals, leads, follow-ups, and results.
- Participate in marketing campaigns, community events, open houses, and promotional activities.
- Monitor local competitors and market conditions and communicate relevant information to leadership.
- Assist in developing strategies to improve brand awareness and increase census/occupancy.
- Maintain a professional and positive representation of Hometown Manor in every interaction.
- Perform other sales, marketing, and community-relations duties as assigned.
Travel Requirements
Regular travel is an essential function of this position.
The Community Relations Representative will be expected to travel between Hometown Manor communities and throughout assigned markets to meet with referral sources, attend events, conduct outreach, and support community occupancy goals.
Travel may include trips of 100 miles or more depending on business needs.
The employee must maintain a valid driver’s license and meet company requirements for operating a company-owned or personal vehicle for business purposes.
Some early morning, evening, or occasional weekend travel/events may be required.
Qualifications
- High school diploma or equivalent required; college degree preferred.
- Previous experience in senior living, healthcare, sales, marketing, business development, or community relations preferred.
- Strong interpersonal and relationship-building skills.
- Excellent verbal and written communication skills.
- Comfortable initiating conversations and developing new professional relationships.
- Ability to work independently and manage a travel-based schedule.
- Strong organizational and follow-up skills.
- Ability to prioritize multiple communities and referral relationships.
- Basic computer and technology proficiency.
- Valid driver’s license and acceptable driving record required.
- Must be able and willing to travel regularly.

Physical & Work Requirements
The employee must be able to travel frequently by vehicle, enter and exit a vehicle repeatedly throughout the workday, attend community events, tour facilities, and occasionally transport or set up marketing materials.

Pay: $25,000.00 - $35,000.00 per year
