Psychiatric Mental Health Nurse Practitioner (PMHNP) 

 

Hometown Health Center (HHC) is seeking a compassionate and experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our integrated care team. This position offers a unique opportunity to provide critical behavioral health services to individuals across the lifespan in a supportive, community-based setting. Full-time or part-time options are available.

About the Role

 

The Psychiatric Mental Health Nurse Practitioner (PMHNP) provides direct psychiatric care, including evaluations, medication management, and ongoing treatment to individuals with a wide range of mental health conditions. While this role supports patients across the lifespan, we are specifically seeking a provider with a strong interest and experience in adult mental health care to complement our pediatric-focused PMHNP.

 

Key Responsibilities

  • Conduct psychiatric evaluations and mental health assessments
  • Develop individualized treatment and medication plans
  • Prescribe and manage psychotropic medications in accordance with current best practices
  • Collaborate with behavioral health providers, primary care clinicians and support staff to ensure coordinated care
  • Monitor treatment response and adjust care plans as needed
  • Maintain accurate, timely and confidential documentation
  • Provide crisis intervention and consultation as needed

 

Minimum Qualifications

  • Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
  • Current Nurse Practitioner license in the State of Maine
  • Active DEA license with authority to prescribe Schedule II–V substances
  • CPR certification (or willingness to obtain)

 

Preferred Qualifications

  • Experience in a community health or outpatient mental health setting
  • Comfortable and confident treating adult patients, with flexibility to support all ages as needed
  • Strong communication and interpersonal skills
  • Commitment to trauma-informed, patient-centered and culturally competent care
