Role Description
If you're passionate about neuro physical therapy and looking for something that not only gives freedom and flexibility to your patients, but to YOURSELF as well, then Homestretch Neuro PT welcomes you! Homestretch is currently hiring for both per diem and full-time Neuro PT roles. In this position, you’ll serve as a primary physical therapist to your client/patient, helping them to discover movement beyond day-to-day living and finding more freedom in life. Your role will emphasize “stretching” the limits of neurologic health and recovery through intensive exercise/activity, personalized treatment planning, and going out to real-world settings to practice real-world skills (such as going with the client to the grocery store, helping them re-learn to swing a golf club at the driving range, etc.). Beyond impairment-based care, our Mobile Physical Therapist will also provide fitness and wellness services, particularly for those with chronic neurological conditions.

Essential functions
- Provides a comprehensive client evaluation at the Discovery Visit
- Provides direct 1-on-1 physical therapy treatment and/or fitness and wellness services at follow-ups
- Collaborate with client for progression of community-based treatment/exercise sessions in outdoor or other public settings
- Completes JaneApp documentation in accordance with company guidelines
- Manage and schedule client follow-up visits
- Provide availability for client to reach physical therapist as needed outside of direct treatment hours
- Identifies the client’s need for other disciplines and services and initiates the appropriate referrals. Provides resources for future needs.
- Provides coordination of care by engaging with supervisor regarding client performance and progress, response to treatment, changes in condition, or challenges to care.
- Maintains confidentiality of information relating to the physical therapist-client relationship.

Homestretch Neuro Physical Therapy is a concierge, mobile practice designed to break-free from standard clinical care, allowing our physical therapists more freedom and autonomy. We have a selective screening process with our clientele, allowing our therapists to engage with friendly, collaborative individuals who are strongly motivated to change their lives. We don't waste time with excessive documentation and "jumping through hoops" - our focus is on providing truly kind and personalized care. Required education and experience
- Minimum 1 Year PT Experience
- Experience or training in neuro physical therapy (NCS preferred but not required)
- Preferably at least 1 year Home Health experience (Training provided for PT without home health experience)
- Must have valid professional license to practice PT in the State of California
- Valid CPR License
- Valid Driver's License and your own reliable transportation
Pay range
- $105 per 60-minute Follow-Up Visits
- $130 per 75-minute Discovery Visits
- Up to $100 reimbursable for supplies
- Reimbursed for mileage
Expected hours/schedule
- As a mobile physical therapist, you have the flexibility to organize your client visits around not only their time, but yours as well. This helps create a work-life balance available only to our staff.
- A per diem mobile physical therapist is expected to see a minimum of 2 visits per week.
- Full-time staff are expected see an average of 12-15 patients per week.
Location
- This is a mobile position. You caseload and location of patients will depend on where you live, but we will NEVER ask you to drive unreasonable distances without fair compensation.
