Homeside Hospice, a nurse owned and operated hospice, is looking for a highly motivated, compassionate, and energetic candidate for Full Time RN, Case Manager to work out of Union , Mercer, Somerset and Middlesex counties.
Homeside Hospice provides exceptional quality care by providing physical, emotional and spiritual care during the End of Life Journey.
Hospice Case Manager will be responsible to oversee, provide care and symptom management from Admission. Continually assess patient and family's needs on a patient specific basis and implement comprehensive care plan interventions. Will work closely with other members of the interdisciplinary team to ensure all patient and family needs are met.
Must have a current and valid license to practice professional nursing in the state of NJ. Demonstrates professional competency, ability to assess and respond to the needs of patients and families in varied settings, and detailed organizational skills.
Must be willing to travel multiple counties in NJ, will be compensated for paid tolls and mileage.
Homeside Hospice offers Medical and Dental insurance for employees that work 30 hours or more a week. PTO benefits are pro-rated according to hours worked.
