What are the responsibilities and job description for the Hospice Social Worker position at Homeside Hospice?
Homeside Hospice, a nurse owned and operated company is seeking a Full Time Hospice Social Worker to work with 3 other passionate social workers with 20 plus years of experience. Must be willing to travel to multiple Counties in New Jersey (Union, Somerset, Middlesex, Mercer, Monmouth, Upper Ocean). Collegial atmosphere, with supportive and proactive team, that embraces social work core values, advocacy and the goal of providing the highest level of service provision. On-call responsibilities (including weekends) are on a rotation of once every 5 weeks.
Job duties include but are not limited to;
- Responsible for the delivery of varied social work services to adult (only) hospice patients and their families
- Provide comprehensive initial psycho-social and bereavement assessments, reassessments
- Direct casework/social work services,
- Individual and family counseling as needed, crisis intervention, community referrals
- Engages in advocacy role
- Provides bereavement services
Education: Licensed by the State Board of Social Work Examiners, with a Master’s degree in social work from a graduate school of social work accredited by the Council on Social Work Education, and at least one year of post-master’s social work experience in a health care setting in accordance with N.J.S.A. 45:15BB-1 et seq and N.J.A.C. 13:44G. Motivated to train the right candidate. Must have current LSW, or LCSW.
