About the Role
At HomeMD, our Mobile Nurse Practitioners deliver primary and specialized care directly to seniors in assisted living communities. Instead of patients traveling to clinics, care is brought to them—supporting comfort, continuity, and timely medical attention.
This role involves traveling within a defined service area, typically up to 45 miles from home, with occasional extended travel up to 60 miles once or twice per month.
What You’ll Do
Patient Care & Assessments
- Conduct routine exams, health screenings, and follow-up visits
- Monitor chronic conditions and identify changes in patient health
Chronic Condition Management
- Manage conditions such as diabetes, hypertension, arthritis, dementia, and heart disease
- Adjust treatment plans and medications as clinically appropriate
Medication Management
- Review medication effectiveness, interactions, and side effects
- Coordinate with pharmacists and providers
- Educate patients and caregivers on medication use
Preventive & Wellness Care
- Administer vaccinations and recommend preventive screenings
- Educate residents and staff on wellness, safety, and fall prevention
Palliative & End-of-Life Care
- Provide compassionate, patient-centered care focused on comfort and quality of life
Acute & Telehealth Support
- Address urgent medical needs such as infections, falls, or respiratory issues
- Televisit support available, particularly for part-time providers
Care Coordination
- Collaborate with physicians, specialists, caregivers, and internal teams
- Support continuity of care across settings
Additional Services
- Behavioral health support with access to a BHI team
- Wound care and pressure injury management, with advanced wound care resources available
- Podiatric and mobility-related care as applicable
How the Role Is Supported
- Medical assistants coordinate scheduling and appointments
- Remote support teams assist with orders, documentation, and follow-ups
Qualifications
- Master’s or Doctorate degree in Nursing
- Board-certified Nurse Practitioner
- Active Michigan NP license
- Experience in geriatrics, primary care, chronic disease management, urgent care, or palliative care preferred
