Job Purpose:
- Provides nursing services in the patient’s home in accordance with agency standards and applicable laws and regulations. Ensures that patients receive the highest possible quality of care.
Key Responsibilities:
- Ensure quality and safe delivery of home health care services.
- Implement current nursing practice to meet the needs of patients.
- Fulfill additional performance responsibilities as assigned and as necessary to support patient care.
- Promote effective written and verbal communication with patients, families, and healthcare teams.
- Use equipment and supplies effectively and efficiently to provide optimal care.
Required Education:
- Completion of a state-approved practical nursing program
Preferred Education:
- Licensure in Practical Nursing
Required Experience:
- Experience in providing nursing services in a home care setting
- Familiarity with applicable laws and regulations governing home health care
- Proven track record of delivering high-quality patient care
