Homebase Medical is growing in the San Fernando Valley, CA area. We are currently searching for an experienced full-time Advanced Practice Clinician (Physician Assistant or Nurse Practitioner) to join our expanding house call practice. You will be responsible for primary care in the home setting, Board and Care, and Assisted Living Facilities.

Key Duties
- Conduct Annual Wellness Visits focused on patient education
- Deliver Chronic Care Management to improve long-term outcomes
- Conduct Transitions of Care visits/assessments
- Perform comprehensive in-home visits including medication review & chronic illness management and medical history updates & physical exams
- Labs, diagnostic screenings, and post-hospital follow-ups
- Primary care management & palliative care services

- Actively support the achievement of Homebase Medical’s Vision and Goals.
- Active Nurse Practitioner or Physician Assistant license in the State of California
- Board certification (Nurse Practitioner or Physician Assistant)
- DEA licensure
- Prior experience strongly preferred (but passion and commitment are just as important)
- Pay Rate: $140,000 - $150,000 annual base salary plus bonus
