Position Summary
The Home Health Aide (HHA) provides hands-on personal care, companionship, and household assistance to clients in their homes. HHAs ensure clients remain safe, comfortable, and supported while following the care plan created by the RN Supervisor.
Key Responsibilities
Personal Care
- Assist with bathing, grooming, dressing, and toileting
- Provide safe transfers (including use of Hoyer lift if trained)
- Assist with ambulation and mobility
- Perform skin care and basic hygiene
Companionship & Emotional Support
- Engage clients in conversation and activities
- Provide encouragement, comfort, and emotional stability
- Maintain a warm, respectful, and professional presence
Home Support
- Light housekeeping (laundry, dishes, tidying)
- Meal planning and preparation
- Grocery shopping or errands (as allowed by supervisor)
Health-Related Tasks
- Monitor and report changes in client condition
- Follow care plan and instructions from RN Supervisor
- Assist with medication reminders (no medication administration unless certified/allowed)
Communication & Documentation
- Clock in/out accurately
- Report incidents, falls, or concerns immediately
- Maintain privacy and comply with HIPAA guidelines
- Complete required forms (timesheets, progress notes, etc.)
Qualifications
- Current NJ HHA, CNA, or PCA certification
- CPR certification preferred
- Experience with elderly or disabled clients
- Ability to lift 30–50 lbs if needed
- Must pass background check
- Reliable transportation

Work Schedule
