As a Home Health Occupational Therapist, you will:
- Complete in-home Occupational Therapy evaluations and reassessments
- Develop individualized treatment plans to improve independence with Activities of Daily Living (ADLs)
- Provide rehabilitation for patients recovering from illness, injury, surgery, or hospitalization
- Recommend adaptive equipment, home modifications, and fall prevention strategies
- Educate patients and caregivers on safety, mobility, and functional independence
- Collaborate with physicians, nurses, Physical Therapists (PT), Speech-Language Pathologists (SLP), and the interdisciplinary care team
- Document patient progress accurately using EMR while maintaining Medicare and agency compliance
- Current Arizona Occupational Therapist (OT) license
- Graduate of an ACOTE-accredited Occupational Therapy program
- Minimum one year of Occupational Therapy experience (Home Health preferred)
- Strong communication, organization, and time management skills
- Experience with EMR documentation
- Valid driver's license, reliable transportation, and current auto insurance
