Registered Nurse (RN) Case Manager
Salary: $70,000–$85,000 (based on experience)

Responsibilities:
- Conduct comprehensive patient assessments to develop and implement individualized care plans
- Coordinate with healthcare teams to ensure effective managed care and discharge planning
- Provide compassionate and professional nursing care to hospice patients in your assigned territory
- Maintain compliance with HIPAA regulations while managing patient information
- Provide education and support to patients and families regarding treatment options and care processes
- Utilize strong communication skills to facilitate collaboration among multidisciplinary teams
- Monitor patient progress and adjust care plans as necessary to optimize outcomes
- Travel within a designated territory to visit patients
- Participate in interdisciplinary group/team meetings
- Participate in on-call rotation as needed
Requirements:
- Current RN License in the State of Texas (required)
- Valid Driver’s License and current auto insurance
- Reliable working vehicle (daily travel required)
- At least 1 year of RNCM Hospice experience as RNCM and Admission Nurse
