What are the responsibilities and job description for the Caregiver (In-Home) position at Home Matters of North Denver?
Position Summary
Caregivers provide direct care and support to Home Matters clients under our Purpose Driven Care (PDC) and Nurse Guided Care model. Responsibilities must be performed in accordance with Colorado Home Care regulations and agency policy. Caregivers receive an individualized service plan for each client and are expected to follow it precisely.
Minimum Qualifications
21 years of age
Minimum 1 year verifiable caregiving experience
English proficiency for communication, documentation, and safety
Successful completion of background and reference checks
Physical ability to lift up to 50 lbs safely and perform required mobility tasks
Required sensory ability (clear vision, hearing, and speech)
Reliable transportation and ability to travel 15–50 miles per shift
Mandatory attendance for required in-person trainings and meetings at the Brighton, CO office
Essential Job Functions
Personal Care
Assistance with bathing, dressing, grooming, toileting
Safe mobility and transfer support
Basic nail care (cleaning and filing only)
Medication Support
Medication reminders only; no handling or application of medications, creams, or ointments
Homemaking & Safety
Light housekeeping, meal preparation, laundry
Home safety awareness and hazard reporting
Companionship & Engagement
Emotional support, conversation, and purposeful daily engagement
Escorting clients to appointments or outings when authorized in the care plan
Documentation & Reporting
Complete accurate daily visit notes and incident reports
Report all changes in condition to the supervisor or RN immediately
Follow all communication protocols and notify the agency of schedule changes
Professional Expectations
Maintain confidentiality and HIPAA compliance
Adhere to infection control, safety procedures, and all state regulations
Complete agency-required training, orientation, and competency assessments
Represent Home Matters with professionalism, reliability, and compassion
