This full-time, on-site role is based in Napa, CA and involves collaborating closely with caregivers, families, and healthcare providers to coordinate services and monitor changes in client conditions. Daily responsibilities include conducting nursing assessments, administering medications, performing basic treatments within scope of practice, and documenting care accurately. The LVN offers education and support to clients and families regarding health conditions, care routines, and wellness strategies, while promoting dignity and independence.
