Role Description The Licensed Vocational Nurse (LVN) provides direct, in-home clinical and supportive care to senior clients, ensuring safety, comfort, and adherence to individualized care plans. This full-time, on-site role is based in Napa, CA and involves collaborating closely with caregivers, families, and healthcare providers to coordinate services and monitor changes in client conditions. Daily responsibilities include conducting nursing assessments, administering medications, performing basic treatments within scope of practice, and documenting care accurately. The LVN offers education and support to clients and families regarding health conditions, care routines, and wellness strategies, while promoting dignity and independence. The role also includes participation in care planning, quality assurance initiatives, and occasional training or mentoring of caregiving staff.
Qualifications
  • Active California Licensed Vocational Nurse (LVN) license in good standing and current CPR/First Aid certification.
  • Clinical skills in basic nursing care, medication administration, vital sign monitoring, and documentation in accordance with regulatory standards.
  • Experience or strong interest in geriatric care, dementia and Alzheimer’s care, and in-home or long-term care settings.
  • Strong interpersonal, communication, and relationship-building skills to work effectively with clients, families, and multidisciplinary teams.
  • Ability to exercise sound clinical judgment, prioritize tasks, and respond calmly to changes in client status or urgent situations.
  • Commitment to person-centered care, cultural sensitivity, and maintaining client dignity, privacy, and confidentiality.
  • Reliable transportation to client homes in the Napa area and willingness to work a consistent full-time schedule.
  • Preferred: Experience with electronic health records, care coordination, caregiver training, and quality improvement initiatives.
