- Educate patients and their families on health management, medication adherence, and disease prevention strategies to foster self-care and wellness.
- Coordinate discharge planning and facilitate smooth transitions between hospital stays, outpatient visits, or community resources.
- Maintain accurate and detailed medical records while adhering to HIPAA regulations to ensure patient confidentiality and compliance.
- Collaborate with interdisciplinary teams including physicians, therapists, case managers, and social workers to optimize patient outcomes.

Skills
