Duties

• Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews to determine individualized care plans.
• Deliver skilled nursing interventions such as medication management, wound care, IV therapy, and post-acute care procedures in accordance with physician orders.
• Educate patients and their families on health management, medication adherence, and disease prevention strategies to foster self-care and wellness.
• Coordinate discharge planning and facilitate smooth transitions between hospital stays, outpatient visits, or community resources.
• Maintain accurate and detailed medical records while adhering to HIPAA regulations to ensure patient confidentiality and compliance.
• Collaborate with interdisciplinary teams including physicians, therapists, case managers, and social workers to optimize patient outcomes.
• Utilize diagnostic evaluation skills and medical terminology knowledge to interpret lab results and clinical data for effective case management.

Skills

• Critical care experience with a background in ICU or emergency medicine settings enhances your ability to handle complex cases confidently.
• Level II trauma center experience or hospital medicine expertise demonstrates your capacity for managing acute injuries and emergent situations.
• Familiarity with infectious disease care, behavioral health, geriatrics, pediatrics (PICU), or post-acute outpatient clinics broadens your scope of practice.
• Strong knowledge of physiology, medical records management, discharge planning, utilization management, and diagnostic evaluation ensures comprehensive patient care.
• Proven skills in patient assessment, vital signs monitoring, intake procedures, case management, and medical-surgical nursing are essential for success.
• Experience with hospital experience environments—including inpatient units—and outpatient clinics supports seamless care delivery across settings.
