✨ What You’ll Do
- Conduct new client admissions and case openings
- Develop and implement individualized care plans in collaboration with physicians
- Ensure all services comply with NYSDOH and LHCSA regulations
- Oversee and maintain accurate clinical documentation and quarterly reviews
- Provide supervision, coaching, and support to Home Health Aides (HHAs)
- Lead staff education and in-service training programs
- Monitor and uphold state and federal compliance standards
- Active New York State RN License
- Minimum 2 years of home care experience
- Strong knowledge of LHCSA regulations and NYSDOH standards
- Ability to work independently and manage responsibilities efficiently
- Excellent communication and leadership skills
