Role Description This part-time, on-site Registered Nurse role is based in Norwood, MA and focuses on supporting clients in their homes and in the community. The Registered Nurse will conduct client assessments, develop and update care plans, and collaborate with Care Pros to ensure safe, high-quality care. Daily responsibilities include monitoring client health status, managing medications, providing clinical guidance, and documenting care according to regulatory and company standards. The nurse will communicate regularly with clients, families, and interdisciplinary team members to address changing needs, coordinate services, and promote client independence and comfort.
Qualifications
- Active Registered Nurse (RN) license in the state of Pennsylvania and current CPR certification.
- Experience in home health, geriatric care, or a related clinical setting, with strong assessment and care planning skills.
- Ability to collaborate effectively with caregivers, families, and interdisciplinary team members, and to provide clear clinical guidance.
- Strong communication, documentation, and time-management skills, with attention to detail and regulatory compliance.
- Comfort working with older adults, including those with chronic conditions, cognitive impairment, or mobility limitations.
- Proficiency with basic technology for electronic health records and care coordination; willingness to learn new systems.
- Reliable transportation to travel to client locations in and around Norwood, PA.
- Ability to meet physical requirements of in-home care, such as standing, walking, assisting with mobility, and occasional lifting.
- Flexibility to work part-time hours, including possible evenings or weekends based on client needs.
