Home Instead is looking for a compassionate, organized, people focused Case Manager to support seniors and their families throughout Essex County.

Position Responsibilities

As a Case Manager, You Will

• Build and maintain strong relationships with healthcare providers, referral partners, senior service organizations, and community organizations.
• Meet with prospective clients and their families to understand their needs, goals, and care preferences.
• Conduct client consultations and help connect families with appropriate Care Professionals.
• Conduct quality assurance visits and follow up with clients and families to help ensure a high-quality care experience.
• Develop and maintain referral relationships and community partnerships that support company growth.
• Represent Home Instead at health fairs, community events, networking opportunities, and industry meetings.
• Maintain accurate client, referral, and activity information in the CRM.
• Track goals, activities, referrals, and results.
• Communicate effectively with clients, families, Care Professionals, referral partners, and internal team members.
• Collaborate with the office team to identify opportunities to improve client satisfaction and community relationships.
• Perform other duties as needed to support clients, families, and the Home Instead team.
  
  

Qualifications

The ideal candidate:

• Has strong relationship-building and communication skills.
• Is compassionate, professional, and passionate about helping seniors and their families.
• Has experience in case management, healthcare, senior care, consultative sales, business development, referral management, community outreach, or relationship management.
• Is organized, dependable, motivated, and able to manage multiple priorities.
• Is comfortable meeting with people in the community and representing an organization professionally.
• Has strong follow-through and attention to detail.
• Is comfortable using CRM systems, email, Microsoft Office, and other standard computer applications.
• Works well independently while also contributing positively to a team environment.
• Is flexible and able to adapt to changing client and business needs.
• Maintains a professional and respectful approach when working with seniors, families, healthcare professionals, and community partners.
  
  

Schedule and Location

Position: Case Manager

Service Area: Essex County, Massachusetts

Office Locations: North Andover and Peabody

Schedule: Monday-Friday, primarily 8:00 AM-5:00 PM

Work Environment: Office and community-based

Regular local travel throughout Essex County may be required for client meetings, quality assurance visits, networking, and community events.
