- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment the resident’s personal assistance devices
- Monitor patient health by observing for changes in condition, and reporting findings accurately while maintaining HIPAA compliance to ensure confidentiality
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote proper nutrition
- Medication Reminders
- Support patients with cognitive impairments such as dementia or Alzheimer’s by implementing memory care techniques and engaging them in meaningful activities
- Maintain a clean and safe environment by performing light housekeeping tasks including cleaning resident areas and ensuring infection control standards are met
- Document care activities thoroughly in accordance with healthcare regulations and assist in updating personalized care plans regularly
- Prior experience working in assisted living, nursing homes, or home & community care settings is highly preferred
- Strong communication skills for collaborating with healthcare teams, residents, families, and social workers
