We are seeking a compassionate and dedicated NJ Certified Home Health Aide to join our team and provide exceptional in-home care to clients across diverse settings. Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using methods and equipment such as Hoyer lifts.
- Provide specialized care for individuals with dementia, Alzheimer's disease, physical challenges or those requiring hospice or end-of-life care.
- Support patients with medical procedures such as catheter care and basic medical documentation as directed by the policy.
- Assist with meal preparation, light housekeeping tasks including cleaning and laundry to ensure a safe and comfortable living environment.
- Communicate effectively with clients and their families about care plans, progress updates, and any changes in health status.
- Follow home healthcare regulations and safety protocols during patient transfers using proper techniques to prevent injury.
Experience
- NJ Certified Home Health Aide is License required.
- Prior experience is preferred, working in-home, as a nursing assistant, or in assisted living or long-term care settings.
- Demonstrated experience working with geriatric patients.
- Experience providing hospice or dementia care is advantageous.
- Understanding home health care assistance regulations, best practices for client communication and patient personal care assistance.
- Ability to lift 25 lbs. safely when required and utilize patient transfer methods effectively.
