Home In The Heart Healthcare Agency is seeking a compassionate and experienced Registered Nurse (RN) to conduct supervisory assessments and oversight visits for veterans receiving Homemaker/Home Health Aide services.
Service Area: Hardin County and surrounding areas
Key Responsibilities
- Conduct initial assessments, supervisory visits, and reassessments.
- Evaluate client health, functional needs, safety, and home environment.
- Develop and update individualized care plans.
- Provide clinical guidance and oversight to caregivers.
- Communicate with clients, families, physicians, case managers, and VA representatives.
- Complete accurate and timely documentation.
- Ensure compliance with agency, VA, and Tennessee regulations.
Qualifications
- Current, unrestricted Tennessee RN License
- Valid driver’s license and reliable transportation
- Strong clinical assessment and documentation skills
- Excellent communication and organizational skills
- Ability to work independently and travel within the service area
Preferred: Experience in home health, geriatrics, veterans care, case management, long-term care, or community-based nursing.

- Visits scheduled based on client needs and referrals
- Competitive per-visit assessment rate
- Mileage reimbursement may be available per agency policy
