Key Responsibilities
Build and maintain strong relationships with referral sources, including physicians, hospitals, skilled
nursing facilities, and assisted living communities
Promote and educate partners on home care services, clearly communicating value and identifying
client needs
Serve as the primary liaison between referral sources, clients, and internal teams to coordinate care
and ensure a smooth start of services
Maintain ongoing communication with partners to support satisfaction, address concerns, and
strengthen long-term relationships
Represent the company in the community through outreach efforts and in-home visits with
prospective clients
Monitor market feedback and referral activity, providing insights and recommendations to improve
growth and marketing strategies
Stay informed on company policies, procedures, and industry trends; participate in team meetings
and support overall team goals
Qualifications
Sales or business development experience preferred, but not required
Strong interpersonal and relationship-building skills
Excellent communication and organizational abilities
Self-motivated with the ability to work independently and as part of a
team
Healthcare experience is a plus, but not required
