The Role
As our SOC Nurse, you'll complete initial home evaluations for pediatric patients, establish care plans, and set the tone for a safe, high-quality home health experience. You'll be a key touchpoint for families at the most important stage of their care journey.
What You'll Do
- Conduct comprehensive start of care assessments in the home setting
- Develop and initiate individualized care plans based on physician orders
- Educate family caregivers on care needs, equipment (vents, g-tubes, pumps), medications, and safety
- Complete thorough, accurate clinical documentation
- Communicate findings and care plans to the clinical team and coordinating nurseCore
Requirements
Please only apply if you meet all of the following:
- Active Florida RN license BLS certification
- 1 year experience completing Start of Care visits in home health
- Strong clinical documentation and EMR experience
- Valid Drive License & Reliable transportation and willingness to travel to patient homes
- Comfort working with medically complex pediatric patients
Preferences
- Experience with Statewise EMR
- Pediatric home health or private duty nursing experience
