Responsibilities

• Provide personal care assistance including bathing, dressing, grooming, and mobility support to clients with diverse needs.
• Assist with activities of daily living (ADLs) such as eating, toileting, and medication reminders in accordance with care plans.
• Monitor patients’ health by observing vital signs, reporting changes, and maintaining accurate documentation.
• Support clients with mobility using equipment like Hoyer lifts or patient transfer methods to ensure safe movement.
• Prepare nutritious meals and assist with meal planning to meet individual dietary requirements.
• Maintain a clean and safe environment through regular cleaning duties and adherence to health and safety protocols.
• Offer companionship and emotional support, fostering a respectful and engaging atmosphere for clients with dementia, Alzheimer’s, or other cognitive impairments.
• Assist with transportation for appointments or community outings, ensuring safety during travel.
• Follow HIPAA guidelines to protect client confidentiality and uphold residents’ rights at all times.
• Collaborate with healthcare teams to implement care plans tailored to each individual’s needs.

Requirements

• Previous experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
• Knowledge of dementia care, Alzheimer’s care, memory care, hospice care, and neurodevelopmental disorders is advantageous.
• Ability to perform manual handling tasks including heavy lifting and patient transfers safely.
• Certification in CPR and First Aid is required; additional training in medication administration or hospice care is a plus.
• Familiarity with patient observation techniques, vital signs monitoring, HIPAA compliance, and resident’s rights is essential.
• Strong communication skills for effective client interaction and teamwork.
• Experience working with individuals with disabilities or developmental disabilities supports success in this role.
• Ability to handle physically demanding tasks while maintaining compassion and professionalism.
• Valid driver’s license or reliable transportation may be necessary for client outings or home visits.
