Occupational Therapist – Help Patients Rediscover Independence at Home


Home Healthcare, Hospice & Community Services is seeking an Occupational Therapist to help individuals regain independence, adapt to changes in their abilities, and safely participate in the activities that matter most to them - all within the comfort and familiarity of their own homes.


As an Occupational Therapist with HCS, you’ll have the opportunity to look beyond the diagnosis and understand each patient’s goals, routines, environment, and support system. You’ll develop individualized plans of care, provide skilled interventions and education, and collaborate with patients, families, physicians, and an interdisciplinary team to help patients achieve meaningful, realistic outcomes.

Key Responsibilities
  • Assumes responsibility and accountability for the practice of occupational therapy in a manner consistent with the NH State Occupational Therapy Practice Act.
  • Consistently demonstrates competency with technical physical therapy skills according to personal and legal scope of practice.
  • Implements teaching appropriate to the client’s needs and within the context of his or her environment and value system.
  • Practices occupational therapy with respect for individual, cultural and spiritual differences.
  • Coordinates delivery of care along health care continuum utilizing community resources.
  • Communicates effectively and in accordance with agency requirements.
  • Demonstrates teamwork and effective communication to accomplish client, team and agency goals.
  • Uses professional occupational therapy judgment to delegate selected tasks when determined safe and appropriate.
  • Maintains established productivity standard or an average of 4-6 patient visits per day.
  • Promotes an awareness and utilization of HCS services in a positive manner.
  • Uses the Code of Ethics established by the American Speech and Hearing Association to maintain confidentiality, advocate for the client and identify ethical conflicts.
  • Assumes responsibility for professional development and contributes to the professional growth of others.
  • Applies knowledge of clinical and administrative policies and practice procedures as outlined in HCS Policy and Procedure manual.
  • Actively participates in the Continuous Quality Improvement Program by expressing ideas, concerns and solutions to improving the quality of service.
Requirements:
  • Graduate of an approved occupational therapy program or equivalent qualifying education.
  • Current State of New Hampshire license to practice as a Registered Occupational Therapist.
  • One year of professional occupational therapy experience under appropriate supervision, or an equivalent combination of education and experience demonstrating the competencies necessary for the position.
  • Comfortable using electronic systems and mobile technology for clinical documentation and patient information.
  • Valid driver’s license and access to an appropriately registered and insured vehicle during working hours.
