LVN Field Staff Home Health

Affinity Healthcare San Diego Inc. Home Health is seeking a compassionate and adaptable Field Nurse (LVN) to join our team. As an LVN, you will provide high-quality, individualized care to patients in their homes. experience with Finale health software are highly desired.

Responsibilities

• Provide direct patient care in a homecare setting, with a focus on individualized treatment plans.
• Conduct patient assessments, including catheterization, wound care, and monitoring vital signs.
• Collaborate with healthcare teams to ensure optimal patient outcomes and continuity of care.
• Utilize EMR systems, including finale Health, for accurate documentation and communication is a Plus.
• Provide accurate and timely documentation consistent with the plan of care.
• Implement discharge planning strategies to facilitate smooth patient transitions.
• Educate patients and their families on health management and disease prevention.

Qualifications

• Current California LVN license.
• 2 year of recent Home Health experience preferred, with skills in wound care, patient teaching, and medication management.
• Must have a high school diploma, Associate, or Undergraduate degree in Nursing.
• Current CPR/BLS certification.
• Valid driver's license with a vehicle in good working condition, registered, and insured.
• Proficiency in EMR software (Finale Health experience is a plus) and basic computer skills.
• Strong observational, communication, and problem-solving skills.
• Bilingual in Spanish-language is a plus
• We looking for staff in Inland Empire and Ventura

Job Types: Full-time, Part-time, Travel nursing

Pay: $40.00 - $45.00 per hour
