Duties
- Provide attentive patient observation to monitor health changes and report findings accurately using Electronic Health Record (EHR) systems
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
- Support patients with specialized needs such as Alzheimer’s care, dementia care, hospice support, and working with individuals with developmental disabilities or disabilities
- Prepare nutritious meals and assist with cooking while ensuring dietary restrictions are followed
- Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall health and safety
- Support patients during transfers using Hoyer lifts or other mobility aids while ensuring safety protocols are followed
Experience
- Prior experience in home health or community care environments is highly preferred
- Experience working in assisted living facilities or nursing homes is advantageous
- Familiarity with medical terminology, vital signs monitoring, medication administration, and documentation procedures is essential
- Demonstrated ability to provide compassionate patient care for seniors or individuals with disabilities
- Knowledge of dementia care techniques including memory care strategies and Alzheimer’s care best practices
- Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as hospice care or long-term care experience are a plus
- Proven ability to work independently while adhering to safety standards and HIPAA confidentiality requirements
