Physical Therapist Assistant Duties:

• Provides physical therapy services as prescribed by a physician and directed by a licensed P.T. which can be safely provided in the home and assists the physician and P.T. in evaluating patients by applying diagnostic and prognostic muscle, joint and functional abilities tests.
• Observes and records activities and findings in the clinical record and reports to the physician and P.T. the patient’s reaction to treatment and any changes in the patient’s condition.
• Treats patients in accordance with the care plan of the patient as established by the Physical Therapist after the initial evaluation of the Physical Therapist.
• Participates in facetoface patient care conference with P.T. to review progress and modification of treatment for each patient, weekly.
• Contacts the P.T. directly for patient reports and care as needed.
• Submits appropriate written reports of the patient’s progress, status, treatment, etc. at the designated intervals to the P.T. and the Agency.
• Instructs patients and their families regarding physical therapy procedures designated by the supervising physical therapist.
• Functions as a member of the health team aimed at total patient care.
• Attends paramedical and paramedical/nursing in service meeting at designated times.
• Works under the supervision of the Physical Therapist and in accordance with the care plan established for each patient.
• Participates in staff development and inservice education activities with the Agency at least once a quarter
• Communicate and coordinate activity/progress with other members of the team.
• Participates in Performance Improvement Activities.
• Performs other jobrelated duties as assigned.
