Coordinates and directs the care of home patients requiring occupational therapy, providing comprehensive assessment, planning, implementation, and evaluation. Performs evaluations to assess patients' functional abilities, including activities of daily living, and documents results in clinical records. Prioritizes and adapts home care caseloads to meet the changing needs of patients and families. Develops patient-specific care plans with realistic, measurable goals in consultation with patients, families, and physicians. Implements care plans through direct patient care, coordination, delegation, and supervision of the healthcare team. Selects and applies skilled interventions to achieve the best outcomes for patients, reassessing as needed. Educates patients and caregivers on therapy techniques, use of assistive devices, and home programs. Assesses the home environment for safety, infection control, and community resource needs. Monitors and documents patient progress, adjusting care plans as necessary to achieve goals. Participates in interdisciplinary care conferences and quality improvement activities. Communicates effectively with physicians, clinical managers, and other healthcare team members regarding patient status and care needs. Maintains accurate and timely documentation of all assessments, care plans, interventions, and patient progress. Other duties as assigned.Schedule: Full Time
Shift: Monday – Friday, 8:30am – 5pm
Location: Covering the Clermont, Minneola, and Groveland
- Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing
- Home Care Regulations and Third-Party Reimbursement as it impacts care delivery
- Demonstrated knowledge of occupational therapy theory, modalities, treatments, and skills
- Basic computer skills and knowledge of Microsoft Office.
- Must be able to read, write, and speak conversational English.
Education:
- Master's [Preferred]
Field of Study:
- in Occupational Therapy
Work Experience:
- 1 relevant clinical occupational therapy experience [Required]
- Recent, relevant experience in a Medicare-certified home health agency as a case-manager [Preferred]

- Education requirements for this position are determined by the licensing or certification standards set forth by the relevant certifying body. Applicants must possess the level of education mandated for eligibility to obtain and maintain the required license or certification.
