The OT will work with a dedicated team of homecare clinicians of RN's, PT's, Medical Social Workers, LNA's and a long term Manager. Staff are proud to say "I work for Home Health & Hospice Care!" Consider the possibilities!
Responsibilities:
- Develops interventions to support goals of therapy and restore maximum function and strength promoting optimal health and minimal pain.
- Evaluates results of occupational therapy by evaluating by observing, noting, and evaluating patient’s progress; recommends and implements adjustments as needed.
- Determines occupational therapy treatment plans in consultation with physicians and homecare team.
- Assures continuation of therapeutic plan following discharge designing and instructing patients, families, and caregivers in home exercise programs; recommending and providing assistive equipment.
- Documents patient services in measurable goals.
Qualifications:
- Knowledge of the principles, concepts and methodologies required of Occupational Therapy and it's practical application in the treatment of patients at home
- At least one year of experience in the hospital setting, facility or orthopedic center. Homecare experience preferred
- Bachelor's degree and NH Licensure as an Occupational Therapist
- CPR certification
- Valid drivers license, reliable vehicle and auto insurance
- Demonstrated problem solving, empathetic communication and time management skills
- EPIC (EMR) experience a plus!
Required:
- Registered as Occupational Therapist with New Hampshire’s Board of Nursing.
- CPR Certification.
- Satisfactory criminal record.
- Satisfactory motor vehicle record.
