About the Job:
The Intake Support Specialist plays a critical role in coordinating patient referrals and ensuring a smooth intake process for home health and hospice services. This position serves as a key point of contact for referral sources, patients, and internal teams, ensuring accurate information, timely processing, and excellent customer service.
What You’ll Do:
- Manage incoming referrals and ensure accurate, timely processing
- Educate callers on agency services and connect them to appropriate resources
- Verify insurance benefits, authorizations, and payer requirements
- Create and maintain patient records in the EMR system
- Identify missing documentation and follow up with referral sources
- Collaborate with care teams, liaisons, and departments to coordinate admissions
- Route referrals appropriately to support timely start of care
- Deliver exceptional customer service to internal and external partners
- Maintain productivity standards and contribute to a team-oriented environment
Who You Are:
- Detail-oriented with strong organizational and critical thinking skills
- Effective communicator (written and verbal) with a customer-first mindset
- Collaborative team player with a positive, solution-focused attitude
- Comfortable managing multiple tasks in a fast-paced environment
- High school diploma required
- Meets background check requirements
Preferred:
- Current RN with the New Hampshire Board of Nursing
- Minimum of two year experience in medical/surgical nursing
- Minimum of one year experience in medical coding and third-party payer authorizations
- ASN or BSN degree
- Knowledge of Epic EMR
