About the Position
We are seeking a Home Health Physical Therapist (1099 Contractor) to provide skilled therapy services to patients in their homes.
This is an independent contractor role, ideal for therapists who value autonomy and flexibility.
Responsibilities
• Perform Start of Care (SOC), evaluations, reassessments, and routine visits
• PTA handoffs and supervision
• Develop individualized plan of care
• Provide patient and caregiver education
• Complete documentation in agency EMR systems
• Communicate with interdisciplinary team and agency staff
• Maintain professional and timely communication
Expectations
• Maintain timely documentation
• Communicate proactively regarding scheduling and patient updates
• Maintain professionalism with patients, families, and agency partners
• Follow Medicare and home health compliance standards
1099 Contractor Overview
