Duties

- Monitor patient vital signs, observe behavioral changes, and report any concerns related to health or safety in accordance with HIPAA regulations.
- Administer medications and document medication administration accurately following prescribed protocols and care plans.
- Prepare nutritious meals, assist with meal consumption, and ensure proper food safety standards are maintained.
- Support resident during outings or appointments by providing personal transportation assistance and ensuring their safety at all times.
- Implement individualized care plans tailored to need care, memory care, and support for individuals with autism or neurodevelopmental disorders.
- Maintain a clean and safe environment by performing routine cleaning tasks and adhering to infection control procedures.
- Observe patient conditions closely, including vital signs and patient monitoring, to identify any changes requiring escalation or medical attention.
Skills
- Proven experience working with individuals with disabilities, developmental or intellectual disabilities support.
- Strong knowledge of patient personal care assistance, medication administration, first aid, CPR certification, and emergency response procedures.
- Ability to perform heavy lifting safely using proper manual handling techniques.
- Excellent communication skills for effective client interaction and collaboration with healthcare teams while maintaining confidentiality under HIPAA guidelines.
- Experience assisting with ADLs such as feeding, grooming, toileting, and mobility support in a compassionate manner.
- Knowledge of home & community care practices along with experience working in long-term or geriatrics settings is highly valued.
- Demonstrated ability to follow detailed care plans accurately while adapting to individual needs in a dynamic environment.
