NOW HIRING – PERSON-CENTERED PLAN PCP SUPPORT SPECIALIST
Location: Bay City, MI
Pay: $20.00–$22.40 per hour
One Client • Community Living Support • Home & Community-Based Services
We’re seeking a dependable, physically capable individual to provide Community Living Supports CLS for an adult with physical and developmental disabilities through a Self-Determination program.
This position focuses on implementing Person-Centered Plan PCP goals in both the home and community. You’ll work one-on-one with one individual to promote independence, mobility, community participation, and quality of life.
Available Schedule
Person-Centered Plan Goal Shifts
- Monday–Sunday
- 2:30 PM – 6:30 PM 4 hours per shift
- Limited availability — only a few shifts remain open
Night Shift Availability
- Monday
- 10:30 PM – 4:00 AM 5\.5 hours
Estimated Weekly Hours Example:If you work four PCP shifts 4 hours each plus one night shift 5\.5 hours, that totals 21.5 hours per week.
Additional hours may become available for coverage as staffing needs change.
Position Responsibilities
- Assist with Person-Centered Plan PCP goals
- Support community outings and participation
- Assist with transfers using adaptive equipment
- Help with standing programs using a stander
- Assist with gait training using a pacer
- Support sitting balance and therapeutic positioning activities
- Perform Hoyer lift transfers
- Assist with mobility throughout the home and community
- Provide personal care, including brief changes when necessary, including during community outings
- Assist with meals and self-feeding as needed
- Complete required documentation
Physical Requirements
This is an active, hands-on position requiring the ability to:
- Perform Hoyer lift transfers
- Assist with standing and mobility equipment
- Bend, stoop, kneel, and lift safely
- Push and maneuver adaptive mobility equipment
- Assist with personal care tasks in both home and community settings
Qualifications
- Reliable transportation
- Dependable attendance
- Comfortable providing hands-on physical support
- Ability to work independently while following a Person-Centered Plan
- Experience as a DSP, caregiver, CNA, Home Health Aide, or related field preferred
- Paid training provided for the right candidate
