Then we have a job for you! HHCM is looking for bachelor’s prepared Social Workers (or another related field such as Gerontology or Psychology) who will work with a Care Management Team (CMT) consisting of Master’s/bachelor’s prepared Social Worker and Registered Nurse team to provide comprehensive case management services to target population. Home & Health Care Management has been providing health and social services to residents of the north state community for over 40 years. Our President is a Registered Nurse with a PhD in Nursing and Health Care Leadership who values the significant differences Social Workers can make to the health and welfare of clients. Home and Health Care Management is responsible for overseeing the comprehensive care management activities of clients residing in Sacramento and surrounding counties for a contract that the agency has with the Medi-Cal Home and Community Based Alternatives (HCBA) Waiver Program. Our agency manages providers and clients in a 23-county catchment area for the California Department of Health Care Services. Our program has doubled in size since 2018 and we need BSW staff to assist the agency in growing and promoting quality patient care.

HHCM is looking for Full-time BSW’s to work in Sacramento and the surrounding areas Monday-Friday from 8am-5pm.

Responsibilities: 

• Assists the team with finding community resources (i.e., housing; medical equipment and supplies; primary care physician and specialists; essential items like food, clothing, and household goods; medical transportation; and caregivers, etc.) needed to meet the participant’s needs.
• Engages in advocacy efforts to ensure participants receive timely medical services to remain safe in the community
• Responds timely to questions and enquiries from CMT, participants, and other entities for smooth patient care coordination
• Provides education to participants so they can better understand the benefits of services and community supports
• Assists participants with applications (i.e., Medi-Cal; IHSS; Calfresh; and Regional Center, etc.). Explains eligibility requirements; coordinates transportation for participants and other basic needs of support
• Documents all consumer interaction and maintains timely reporting and documentation requirements
• Observes participants behavior; reports significant changes to supervisor or team; documents appropriately
• Daily case conference with CMT about shared participants
• Performs other related duties as assigned.

Experience & Education & Other Requirements: 

• Bachelor’s degree in Social Work or another related field
• Work is performed in a home, community, and/or office environment.
• Transporting clients with personal vehicle is prohibited
• Valid California driver’s license and insurance and ability to drive oneself on all roads & weather conditions.
• Need to provide a clean DMV driving record with no violations. 
• Agency requires all employees to show proof of full COVID vaccination or have an exemption. PPE is provided to all employees
• CPR certificate required within 30 days of hire
• Required to work Monday through Friday, 8:00 am to 5:00 pm with an hour lunch
• Required to report and work out of the Sacramento office

• Knowledge of community and supportive services
• Skilled in assessing and prioritizing multiple tasks, projects, and demands
• Skilled in working within deadlines to complete projects and assignments
• Skilled in establishing and maintaining effective working relations with co-workers and other community providers
• Skilled in operating a personal computer utilizing a variety of standard software (including an electronic medical record desirable)
