At Home Health Care, Inc., we're looking for a Physical Therapist who wants to make Southwest Minnesota their territory while maintaining a schedule that's increasingly difficult to find in healthcare:
Monday through Friday. 8:00 a.m. to 5:00 p.m. No weekends. No holidays.
You'll spend your weekdays traveling throughout Southwest Minnesota, bringing skilled Physical Therapy directly to patients in their homes. You'll experience different communities, changing scenery, diverse clinical situations, and significant professional independence.
Then at 5:00 p.m., your workday is done.

What You'll Be Doing
- Perform comprehensive in-home Physical Therapy evaluations.
- Develop individualized plans of care based on functional goals.
- Provide evidence-based interventions addressing strength, balance, gait, mobility, and independence.
- Identify home safety concerns and fall risks.
- Provide caregiver education and equipment recommendations.
- Monitor patient progress and adjust treatment plans as appropriate.
- Collaborate with nurses, physicians, PTAs, OTs, and other interdisciplinary professionals.
- Complete accurate and timely clinical documentation.
- Travel throughout Southwest Minnesota to meet patient needs.

Required Qualifications
- Active Minnesota Physical Therapist license
- Valid driver's license and reliable transportation
- Willingness to travel throughout Southwest Minnesota
- Strong clinical assessment and decision-making abilities
- Excellent patient and caregiver communication skills
- Ability to independently manage a field-based schedule
Preferred Qualifications
- Home health experience
- OASIS experience
- Experience with medically complex or geriatric patients
- Strong interest in relationship-based, one-on-one care
