This is where nursing starts to feel sustainable again.
Instead of being tied to a unit or schedule you can’t control, you’re in the field, managing your own day, seeing patients one at a time, and making decisions that directly impact outcomes.
You are not just completing tasks. You are leading care, guiding families, and seeing progress happen in real time.
• 1:1 patient care instead of juggling multiple patients at once.
• Flexible, field-based schedule with control over your day.
• More time for clinical thinking, education, and connection.
• Opportunity to see patients improve over time, not just stabilize.
We operate alongside Interim Healthcare, Lifespark, Adara, Aveanna, and Gentiva, but we focus heavily on what matters to nurses.
• Strong communication and support from leadership and clinical teams.
• A locally owned, 30 year organization with long-term stability.
• A culture that values retention, relationships, and teamwork.
• Manage a home health caseload across homes and community settings.
• Complete OASIS assessments and individualized care plans.
• Provide skilled nursing care, education, and coordination.
• Collaborate with physicians, therapists, and interdisciplinary teams.
• Support patients in maintaining independence and safety at home.
• Maintain accurate, timely documentation.
• Active Minnesota Registered Nurse (RN) license.
• 3 years of RN experience, home health preferred.
• Strong clinical judgment and independence.
• Organized, self-directed, and comfortable in a field role.
• Reliable transportation.
If you are looking for a role where you can control your day, focus on patients, and build a long-term career in home health, we would love to connect.
