Job Title: Registered Nurse (RN) Case Manager

Job Type: Full-Time (Monday to Friday 8:00am - 5:00pm) 1 Weekend and on-call availability.
Compensation: $80,000 - $120,000 per year $6000 in Bonuses During First Year of Employment!

Registered Nurse - RN Case Manager Overview:
- Screen and assess clients; develop, implement and review patient care plans, making adjustments as necessary to meet changing needs and goals
- Complete OASIS timepoint visits: admissions, re-certifications, and discharges of clients
- Monitor and evaluate patient care delivery to ensure safe, medically appropriate and cost-effective care
- Facilitate referrals to, and coordination with, other services to meet patient care needs and manage the care transition process between settings
- Communicate and collaborate with the patient’s primary provider and other healthcare and community providers
- Monitor care to assure compliance with the plan of care
- Assess psychological, psychosocial and psycho-spiritual needs of the patient and family
- Provide patient/family education and counseling
- Develop relationships with other home care organizations to ensure patient access to appropriate services
- Create open lines of communication with home care staff
- Ensure quality nursing practices are in compliance with organizational and regulatory standards
- Work with multidisciplinary team to provide quality care that is tailored to the patient’s physical and emotional needs
Registered Nurse - RN Case Manager Qualifications:
- Licensed as a Registered Nurse / RN in the state of Minnesota
- At least three years of nursing experience, home care experience preferred
- Knowledge of home care regulations, standards and quality indicators
- Ability to work collaboratively with other members of the interdisciplinary care team
- Ability to evaluate and interpret a variety of healthcare situations and make sound decisions
- Proficient in use of computer systems and software applicable to job duties
- Strong organizational, assessment and problem-solving skills
- Valid Driver's License and reliable vehicle with current insurance
