Responsibilities Include:
- Making scheduled and as-needed visits to patient homes and facilities
- Managing symptoms, monitoring health changes, and offering wound care
- Supporting emotional needs and educating family members
- Documenting clearly in our EMR system
- Collaborating with a dedicated interdisciplinary team
What You’ll Need:
- MN RN license in good standing
- Experience in hospice preferred (palliative, oncology, or med-surg with EOL experience also welcome)
- Confidence in working independently and managing time well
- Strong interpersonal and documentation skills
- Driver’s license and dependable transportation
